WHAT THE FILE IS One row per practice: 50 practices, ten in each of five U.S. metros (South Florida, Dallas-Fort Worth, Chicago/Cook County, New York City and Los Angeles County). Observations come from public websites and booking menus checked September 29-30, 2026. Practices were the top organic Google Maps results for "med spa" and "Botox" near five neighborhoods in each metro; sponsored results were skipped, and one location per chain was kept within a metro. This is a convenience sample of posted offers, not a city or national average, and not what patients paid. We did not call practices, request private quotes, book appointments or submit patient information. Prices may have changed since the check date. "no" includes sites we could not fully read; it does not mean the practice publishes no price. botox_unit_price The nonmember per-unit price posted for Botox, or for a listing that includes Botox with other brands. For a "starting at" price or a range, the low number. Blank if no such price was found. botox_unit_note starting_at: a floor, not a guaranteed price. range: the low end of a posted range is recorded. grouped_brands: the listing includes other brands with Botox. chain_page: a page shared by several locations; the sampled branch's own price is not separately established. HOW THE GUIDE'S FIGURES WERE COMPUTED 18 of 50 rows have a botox_unit_price, one value per practice. 12 of 18 are between $12 and $15, $15 is the most common value (5 rows), and values run from $9.99 to $18. 38 of 50 rows have price_found_in_inspected_sources = yes. Units of different toxins are not interchangeable, so brands should not be compared by unit price. CORRECTIONS LOG 2026-09-30, before publication: - Ever/Body Williamsburg: removed three per-unit rates that appeared only in page code, not on the visible page. - SkyBalance MedSpa: removed a home-page "$25 off" entry that had been misread; that slide is the Daxxify offer already recorded. - The Fort Lauderdale MedSpa: membership payments described as banked treatment credit; booking source added. - Brows and Botox Med Spa: full booking link. - VIO River North: repaired the Botox page link to match the retained source. - Public export: removed annotations and platform-only labels from URLs. Added full price ranges for Refine and Skin by Lovely, and Mara's additional $14.50 figure, without exposing internal notes. None of these changed a count or a headline figure. 2026-09-30, after publication: - Peace Love Med, Boca Raton: expanded the Square service options and corrected our incomplete top-level reading. Added the 1/2/3-area prices ($249/$499/$749), specialty-treatment prices and follow-up terms. Removed our incorrect statement that the card did not identify the area. Retained the separate $999 hyperhidrosis card with its original qualification. No unit price was derived. The 18-listing count, headline distribution and all five market summaries are unchanged. 2026-09-30, expanded booking-menu follow-up: - Revisited the other 19 South Florida and Dallas-Fort Worth booking routes after the Peace Love Med correction. Inspected 14 service menus, expanding available toxin descriptions/options. Three routes led to request/intake forms. Two remained unverifiable: It's A Secret (security verification) and Dermacare (connection failure). - Added per-row recheck dates, status and notes, and direct booking URLs. Original price-source dates remain. - Clarified Cosmetica's booking deposit, Solea's $0 promotion placeholder, Glo & Glam's cancellation fee, Miss Botox's units-based wording without an explicit $13/unit label, and Hush's conditional add-on prices. - No recorded price amount, unit-listing classification or headline count changed. Unreadable menus are not classified as proof that a practice has no posted prices.
